“I, ________________________________________, am employed by Flagler County Public Schools as a full-time, certified classroom teacher. I acknowledge that Florida Teachers Classroom Supply Assistance Program funds are appropriated by the Legislature for the sole purpose of purchasing classroom materials and supplies to be used in the instruction of students assigned to me
I understand that if I do not provide or keep the receipts, it will be my personal responsibility to pay any federal taxes due on these funds per the IRS Code.
I agree to submit receipts for all expenditures through the ClassWallet platform no later than April 30, 2020.
In, accepting custody of these funds, I agree any unused funds will be returned to the School Advisory Council account of the school at which I was employed at the time of the receipt of the funds.”
I understand and agree when purchasing classroom supplies pursuant to the FL Statute 1012.71, (2) these supplies may not be used to purchase equipment, and (5) the sole purpose of purchasing classroom materials and supplies is to be used in the instruction of students assigned to me.”
The term “classroom teacher” includes certified teachers employed on or before September 1 of each year whose full-time job is the classroom instruction of student in grades K through 12, and full-time media specialists and guidance counselors who serve students in grades K through 12.
